REGISTRATION

Registration fees cover meeting sessions, materials and the following meals:
 Two continental breakfasts, two lunches, beverages and light snacks.

Early Registration Fee - $200 (registration form and payment must be received by September 19, 2008)

Regular Registration Fee - $250 (registration form and payment received after September 19, 2008)

Completed registration forms should be returned to the address below along with the appropriate registration fee (check, credit card or money order only) or purchase order. Checks and money orders should be made payable to Spaulding for Children (Federal I.D. # 38-1871660).
